What is ZagMeter Health Risk Assessment?

The goal of this health questionnaire is to assess your current health & wellness status. Based on your response, a report will be generated that will score your current health, as well as make general recommendations for improving your overall physical, mental, and emotional health.

The report will also:

  • Identify the areas of good health and wellness and encourage you to continue to keep these healthy habits and activities
  • Identify your potential health risks
  • Recommend screening tests
  • Help you track your progress in real time

How long will this assessment take?

What do you need before your start (recommended)

Before starting, you should gather the following information, if you have it:

  • Weight and Height
  • Neck and hip measurements (hip measurements for women only)
  • Blood Pressure
  • Blood sugar
  • Total cholesterol
  • HDL cholesterol

However, if you don’t have all information, you can fill in as much as you can, and come back to finish it later. You will have two weeks to complete the questionnaire(you must allow cookies in your browser for this functionality to work)
